Évaluation des pratiques de prise en charge des cancers du sein localisés HER2 positif en Côte-d’Or : étude rétrospective à partir du registre des cancers du sein et gynécologiques de Côte-d’Or

Abstract : Background : Standard care for early HER2-positive breast cancer is based on surgery associated with chemotherapy and HER2-targeted therapy (Trastruzumab). Before 2019, the chemotherapy was administered before surgery only to allowed a breast-conserving surgery by downsizing the tumor. Since, neoadjuvant therapy has become the standard of care because it provides information about tumor response in vivo and patient who did not achieve a pathological complete response (pCR) will benefit of TDM-1 after surgery. The aim of these study was to describe the clinical practices for those patients and to identify a change since 2019. Method : We performed a retrospective study based on the breast and gynecologic cancer register of Côte-d’Or involving the patients with an early HER2-positive breast cancer between 2005-2015 and 2019-2020. Results : From 2005 to 2015 we included 480 patients. The neoadjuvant chemotherapy rate was 14% and the pCR rate was 54%. Association of taxane and Trastuzumab was the main chemotherapy protocol for neoadjuvant therapy (65%). The overall survival was 83% with a statically significant higher rate for patients who achieved pCR (HR= 0,2 IC95% (1.06-23.7), p= 0,02). From 2019 to 2020 we included 63 patients. The neoadjuvant chemotherapy rate was 73% and the pCR rate was 37%. Association of anthracycline, taxane and Trastuzumab was the main chemotherapy protocol for neoadjuvant therapy (91%); after surgery the majority of patients received taxane and Trastuzumab (75%). Conclusion : We observed a real change in clinical practice for early HER2-positive breast cancer both in therapeutic sequence and chemotherapy protocol since 2019.
